Abstract: A sífilis se insere no mundo enquanto grave problema de saúde pública e uma das principais preocupações sobre as dificuldades no controle é a infecção de mulheres em idade reprodutiva, que pode acarretar a sífilis congênita por meio da transmissão vertical. Considerando a relevância da sífilis na gestação e da sífilis congênita no campo da saúde pública e a necessidade de conhecer com mais propriedade a realidade local, objetivou-se neste estudo analisar os aspectos epidemiológicos da sífilis gestacional e congênita no Estado da Paraíba, a partir de dados do Sistema de Informação de Agravos de Notificação (SINAN) no período de 2008 a 2017. Trata-se de um estudo de delineamento transversal, de abordagem quantitativa, com utilização de dados secundários, a partir do SINAN com o estudo de 3.434 notificações. A partir das fichas de notificação/investigação da sífilis congênita e em gestante foram coletadas informações demográficas, socioeconômicas, clínicas e laboratoriais. A análise descritiva dos dados foi realizada por meio do software R 3.5.2 e dois modelos de regressão logística foram ajustados para a análise dos fatores associados ao tratamento da sífilis, considerando o nível de significância (¿) de 5%. Os resultados apontaram uma tendência de ampliação da incidência ao longo dos anos, tanto para a sífilis em gestante como para a sífilis em crianças. Sobre a sífilis em gestantes, a maior parcela das mulheres tinha idade entre 21 e 30 anos, com notificação no terceiro trimestre de gravidez, de classificação clínica primária, com teste não treponêmico reagente e tratadas com benzilpenicilina. Quanto à sífilis congênita, a maior parte dos casos teve notificação registrada em João Pessoa (56,34%), em 84,7% das notificações as mães realizaram pré-natal e 47,31% receberam o diagnóstico da sífilis materna durante o pré-natal. Nas crianças, dados laboratoriais exibem resultado reagente em 75% dos casos para o teste não treponêmico. Quanto à evolução, em 84,3% dos casos a criança permanece viva, enquanto apenas 2,3% são vítimas de óbitos por sífilis congênita. O cenário da sífilis em gestante e congênita na Paraíba evidencia uma progressiva ascendência no período investigado e esse panorama suscita o urgente fortalecimento de medidas de promoção da saúde, estimulando a corresponsabilização da população na adoção de medidas preventivas para a doença. Não obstante, os órgãos de saúde devem ampliar as ações e serviços voltados à saúde sexual e reprodutiva, sobretudo no que tange a assistência pré-natal, favorecendo melhores condições de acessibilidade da população.
Abstract: Syphilis is inserted in the world as a serious public health problem and one of the main concerns about difficulties in control is the infection of women of reproductive age, which can cause congenital syphilis through vertical transmission. Considering the relevance of syphilis in pregnancy and congenital syphilis in the field of public health and the need to better understand the local reality, the aim of this study was to analyze the epidemiological aspects of gestational and congenital syphilis in the State of Paraíba, from data from the Notifiable Diseases Information System (NDIS) from 2008 to 2017. This is a cross-sectional study, with a quantitative approach, using secondary data, based on NDIS with the study of 3,434 notifications. From the notification/investigation forms of congenital syphilis and in pregnant women, demographic, socioeconomic, clinical and laboratory information was collected. The descriptive analysis of the data was performed using the software R 3.5.2 and two logistic regression models were adjusted to analyze the factors associated with the treatment of syphilis, considering the significance level (¿) of 5%. The results showed tendency to increasing incidence over the years, both for syphilis in pregnant women and for syphilis in children. Regarding syphilis in pregnant women, most women were aged between 21 and 30 years old, with notification in the third trimester of pregnancy, of primary clinical classification, with non-treponemal reagent test and treated with benzylpenicillin. As for congenital syphilis, most cases were registered in João Pessoa (56.34%), 84.7% of the notifications the mothers received prenatal care and 47.31% received the diagnosis of maternal syphilis during prenatal care. In children, laboratory data exhibit a reagent result in 75% of cases for the non-treponemal test. As for evolution, in 84.3% of cases the child remains alive, while only 2.3% are victims of deaths from congenital syphilis. This scenario of syphilis in pregnant women and congenital in Paraíba shows a progressive ascendancy in the investigated period and this panorama rise the urgent strengthening of health promotion measures, stimulating the co-responsibility of the population in the adoption of preventive measures for syphilis. Notwithstanding, health agencies should expand actions and services aimed at sexual and reproductive health, especially with regard to prenatal care, favoring better accessibility conditions for the population.
