Title: Fatores associados à retirada regular da terapia antirretroviral na Cidade de Santos

Abstract: Introdução: O Brasil e o mundo ainda sofrem com a notificação diária de novos casos de pessoas infectadas pelo HIV; contudo, também é de conhecimento universal a evolução no conhecimento adquirido ao longo dos anos acerca da infecção e seu tratamento. O Brasil possui como protocolo instituído pelo Ministério da Saúde (MS) o ¿Testar e Tratar¿, que confere à PVHIV após o diagnóstico a recomendação imediata da terapia antirretroviral, independentemente de sua condição clínica. Essa estratégia reflete a ¿Meta 90 90 90¿ que estipulou até o ano de 2020 que 90% das pessoas que vivem com HIV conheçam seu estado sorológico, 90% dessa população esteja em tratamento e que 90% esteja em supressão viral. Assim, o Brasil conquista posição de destaque entre outros países em desenvolvimento quanto ao enfrentamento da epidemia de HIV, ação que, além de beneficiar indiscriminadamente toda população, contribuiu diretamente para o rompimento da cadeia de transmissão do HIV. Objetivo: Analisar os fatores associados à retirada regular de antirretrovirais dos pacientes matriculados no Serviço de Atendimento Especializado em HIV da cidade de Santos nos anos de 2014 a 2018. Método: Estudo quantitativo de recorte transversal, realizado nos anos de 2014 a 2018 por meio da análise de prontuários do Serviço de Assistência Especializada ¿ SAE em HIV/AIDS da cidade de Santos. Para realizar a análise do estudo foram avaliados o indicador de tempo e a quantidade de medicamentos dispensados pela farmácia do SAE ¿ Santos extraídos do banco de dados SICLOM. O intervalo adotado para essa determinação foi dos primeiros 180 dias a contar da primeira retirada do paciente, desde que tenha sido efetuada até o mesmo período após sua entrada no serviço, seguindo assim os objetivos desta pesquisa. O indicador metodológico utilizado foi baseado na Proporção de Dias Cobertos (PDC ¿ Proportion of Days Covered). Resultados: Dentre os pacientes participantes do estudo 84% possuíam retirada regular da terapia antirretroviral. Em relação ao perfil da população do estudo, pudemos observar que na variável sexo (1.072) o masculino apresenta 79% em relação ao sexo feminino; observa-se que 89% das pessoas são adultos com idade entre 25 e 60 anos; e quanto à cor da pele (979) 1/3 da população é parda. Quanto ao nível de escolaridade (1.032) da população 67% das pessoas possuem nove anos ou mais de estudo; quanto ao estado marital (1.025) 60% da amostra declararam ser solteiras; e em referência à categoria de exposição (1.017) metade autorrelatou ser heterossexual. Em relação às condições clínicas, 65% das PVHIVs apresentaram resultados dos linfócitos CD4 ¿350 células/mm³ e 57% estavam com o resultado para exame de carga viral com limite <50 cópias/mL. Ao analisar o tempo de diagnóstico, os resultados demonstraram que 87% das pessoas tiveram o diagnóstico após o ano de 2014. Quanto ao esquema terapêutico utilizado pelos pacientes 79% das pessoas usavam outras terapias que não apenas a 3x1; 82% efetuaram pelo menos duas ou mais retiradas. Conclusão: As características pessoais dos pacientes são fatores que podem influenciar no processo da retirada da terapia antirretroviral e em consequência o nível de adesão ao tratamento. É importante identificar o perfil dos pacientes vivendo com HIV para conhecer suas características pessoais e assim direcionar os esforços mediante suas necessidades. A retirada da terapia antirretroviral pôde ser associada a uma escolaridade da população, porém ainda há relevância em analisar outros fatores que possam melhor compor o perfil dos indivíduos em retirada regular da terapia.
Abstract: Background: Brazil and the world still struggle with the daily notifications of new cases of HIV infections; however, the evolution in the knowledge acquired over the years about the infection and its treatment is also universally known. In Brazil, the protocols instituted by the Ministry of Health (MS) - known as ¿Test and Treat¿ - allows people living with HIV (PLWHIV) to be immediately diagnosed; and if confirmed HIV positive, to receive antiretroviral therapy regardless of their clinical condition. This approach reflected in the implementation of the so-called ¿Goal 90 90 90¿; which aimed that 90 percent of the population with HIV to know their diagnosis by 2020; 90 percent of the population to undergo treatment; and 90 percent of the population to be on viral suppression. Thus, Brazil gains a prominent position among developing countries in terms of tackling the HIV epidemic; therefore, the protocol resulted in the general benefit for the entire population, and directly contributed to the interruption of the transmission chain of HIV. Objective: To analyze the factors associated with the regular collection of antiretroviral therapy (ART) from patients who attended to the Specialized HIV Care Service in the city of Santos from 2014 to 2018. Method: Quantitative-based cross-sectional study carried out from 2014 to 2018 using the records of the Specialized Assistance Service - SAE on HIV/AIDS in the city of Santos. In order to carry out the analysis of the study, the time indicator and the quantity of drugs dispensed by the SAE - Santos pharmacy were obtained from the SICLOM database. The interval used in this case was the first 180 days (from the patient¿s first collection), provided that it was carried out until the same period after SAE registration. The methodological indicator used was based on the Proportion of Days Covered (PDC). Results: Among the patients participating in the study 84% collected the antiretroviral therapy (ART). In relation to the profile of the study population, there were 79% more males than females (total population: 1,072); 89% of population were adults between 25 and 60 years; and as for skin color (979) 1/3 of the population was considered brown. As for the level of education 67% of the population (1,032) had nine years or more of any level of education; 60% of the population (1,025) was reported as single; and in reference to the exposure category (1,017) 50% reported to be heterosexual. Regarding the clinical condition, 65% of the PLWHIV presented had CD4 lymphocyte count ¿350 cells/mm³; and 57% had viral load examination results with a limit of <50 copies/mL. When analyzing the time of diagnosis, the results showed that 87% of people were diagnosed after 2014. As for the therapeutic methods utilized, 79% of the patients used other therapies than the combination 3x1; 82% collected the drugs at least twice in the last 180 days. Conclusion: The characteristics of the patients are factors that can trigger the collection of antiretroviral therapy at the SAE; and consequently, the level of adherence to treatment. It is important to identify the profile of patients with HIV in order to know their personal characteristics and direct efforts according to their needs. The collection of antiretroviral therapy is associated with the level of education; however it is relevant and necessary to analyze other factors that build the profile of those selected for regular therapy collection.
